Abstract
This qualitative single-case study investigated the inefficiency and limited accessibility of telemedicine services within a selected healthcare organization located in Baltimore, Maryland. The purpose of this study was to investigate how telemedicine services can be improved to enhance healthcare outcomes for patients and their overall quality of life. The data were collected using semi-structured interviews with 16 participants, supported by fieldnotes, and publicly available documents, and analyzed using reflexive thematic analysis. Methodological triangulation was used to validate the findings to ensure credibility and consistency. The research indicated that telemedicine enhances access to healthcare and supports continuity of care, especially for those experiencing obstacles related to logistics and mobility. Nevertheless, the research also indicated various barriers hindering the efficiency of telemedicine, such as digital inequities, unreliable technological infrastructure, inefficient workflows, limited digital literacy, workload challenges on providers, and inconsistent organizational implementations. Seven interdependent themes emerged from the analysis which explained that telemedicine performance is affected by the interplay of socioeconomic, technological, organizational, and workforce factors within the healthcare environment. The research concluded that the improvement of telemedicine services requires an organized, system-level strategy that incorporates technological infrastructure, workforce readiness, standardized workflows, communication practices, and organizational support. The findings also support the integration of telemedicine into broader medical care delivery systems through hybrid care models that merge virtual and in-person services depending on clinical appropriateness. This research adds value to professional business practice by revealing evidence-based strategies that can reinforce sustainable implementation of telemedicine, enhance operational efficiency, and encourage equal access to healthcare services.
